Care Coordination

What is Coordination of Care? And What Does it Mean? 

Coordination of care is the process of ensuring that all the professionals involved in your mental health and well-being—such as therapists, psychiatrists, primary care doctors, or other specialists—are communicating and working together to provide you with the best support.

This may include sharing relevant information, treatment plans, or progress updates (with your consent) to create a more comprehensive and effective approach to your care. Coordination of care helps ensure that all aspects of your health are considered and that you receive consistent, well-rounded support.

Surviving to Thriving Therapy provides limited coordination of care with other members of your healthcare team, which may include your disability advocate, case manager, vocational rehabilitation counselor, physical therapist, primary care doctor, dietitian, or other mental health providers. 

If you request your therapist to coordinate care with other professionals involved in your treatment, the therapist will assess whether this will require more than 5 minutes of email correspondence. Should the coordination require additional time, a fee will be charged for the service. The fee for care coordination is $25 per 15 minutes.

The fee for coordination of care compensates the therapist for their time spent consulting with other professionals and for the expertise and insights provided to your broader care team.
